Rhode Island Discipline Report

Overview

 

This guide will outline each data element that will be reported to the Rhode Island Department of Education as part of the Discipline Report and define where the data will be stored in SchoolMAX.  The Discipline Report send to the state will only include the incidents that have a response code that defined by the state are reportable.  These reasons are:  Out of school suspension, In-School suspension, and Alternate Program Placement. 

 

This guide is not intended to override any directives given regarding your districts policy as to other discipline data that should be recorded in SchoolMAX, but ONLY to identify what is necessary to meet the Discipline reporting requirements. 

 

CE220 Family Member Screen

 

Lunch Status Field

 

 

 

The following data elements will all originate from CE220 Family Member Screen.

·                 State Assigned Student ID – State ID Field

·                 Student Gender – M/F

·                 Student Date of Birth – Birthdate Field

·                 Student Race – Valid values are 1-7.  The numeric codes will map to the State Reportable alpha codes.

·                 Student Lunch Status – Valid values are:  0 = P (Paid); 3 = P (Paid); 8 = (F) Free, 9 = R (Reduced)


ST208 Student Enrollment Screen

 

 

The following data will all originate from ST208 Student Enrollment – School screen.  If enrollment is completed on the ST210 screen, there is no difference in the data element origin.

 

·                 Student Grade – Grade field


SP205 Special Education Record – Student IEP Status

 

 

If a student has, at anytime during the school year, received special education services under an IEP, the student must be report with an IEP status of Yes.  The export process will report a “Y” in the Student IEP Status data field for records that have a “4” Receiving Services in the Eval line.  Your school district may require that additional special education data be maintained other than the minimum to meet the state reporting requirements.  A student must have the following to be reported as having an IEP. 

 

  • Sch  Yr - A record for the reporting school year, i.e. 2004
  • Date – The date the student began receiving services.
  • Eval – Eval MUST equal 4 – Receiving Services

 

 


EV205 Student Intervention Entry

 

 

The EV205 screen allows the discipline event to be recorded on one screen.  If the event does not have weapons involved and the Event Code is not 33 – Other, all data can be keyed into this screen except for the duration.

 

·       Infraction – Event field

·       Date of Infraction – Date field

·       Discipline – Response field


EV230 Response

 

 

The number of days a student has been assigned to OSS, ISS, or an Alternative Program Placement must be entered on the EV230 Response screen in the Actual Days field.

  • Duration – Actl Days data field

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

EV210 Incident

 

 

If the discipline incident involved a weapon that is reportable, that information should be entered in the Weapon Used field on the EV210 screen.  The following weapons have been defined as reportable by the state.

 

 

 

If the Infraction reported is 33 – Other, it is required that a “Reason” be reported.  The data element “Reason” should be entered in the Other Factor data field on EV210.  It is likely that the table behind this data field will have to be updated to include the “Reason” for the infraction.  The Reason should be added to the EV280 “Other Factor” code table.  The data that will be exported in the Discipline report will be what is contained in the “State Code Field”.

 SY005 – District - Reporting District Code

 

The reporting district will be pulled from the State Dist # data field.

 

SY010 – School – School Code

 

The school code that will be reported for each student will be based the value found in the State School # data field.
