The Home File



Because you know your child best, you have valuable information to share. Having a file on your child helps you share the information needed by everyone involved in the planning decisions.

Step #1 - Gather all materials…
such as health records, school reports, and work samples (if appropriate). Ask for copies of all reports from doctors and other professional who have worked with your child.

Step #2 - Organize a file…
preferable into a 3-ring binder, so you can add or transfer information. Dividers will help you easily find the information and keep it up to date.

Step #3 - Record…
all calls made to obtain information or services. Include the name and title of person you spoke with, the purpose of your call and its results, as well as the date you called and the date you were call back. Get a commitment for a follow-up phone call.

Step #4 - Record…
the date, purpose, and outcome of all visits, appointments, and conferences.

Step #5 - Observe…
your child for brief periods and record in your notebook what you see and hear. Include the date and location of your observation.

Step #6 - Write down key terminology…
that relates to your child's diagnosis and learning needs. Get explanations and examples from professionals.

Step #7 - Keep a copy…
(dated) of all correspondence regarding your child's education and health
