IMMUNIZATIONS

In accordance with Chapters 21, 22 and 8 of Title 16 of the General Laws of Rhode Island, as amended, and rules and regulations adopted by the Departments of Health and Education, immunization requirements to ensure adequate immunizations of all school-age children in Portsmouth shall include the following:

DPT -  (Diphtheria, Tetanus, Pertussis) - 5 doses

TOPV  - (Oral Polio) - 4 doses

MMR  -  2 doses after 12 months of age or singularly: Measles - 1 dose after 15 months of age  

Mumps - 1 dose after 1 year of age

Rubella - 1 dose after 1 year of age

TB - Test required only for those children of known exposure.

Proof of immunization is required for entrance to school. As an alternate, a certification from a licensed physician stating, that a child is not a fit subject for immunization for medical reasons, or a certificate signed by the parent or guardian stating that such immunization is contrary to the childs religious belief exemption, a child will be excluded from school until immunization or alternate certification is received.
