Dear Parent/Guardian:
Rhode Island State Law requires that all students entering school must present a complete immunization record signed by the doctor, or doctor’s designee. No child shall be admitted to a Portsmouth school without proper certification of immunization. A complete immunization record includes the following:
1. DPT (Diphtheria, Peruses, and Tetanus) – 5 doses, with the fifth dose given after the age of four. (If your child is already age four and has had only 3 doses, 4 doses are allowable as long as the last one is given after age four.)
2. Polio – 4 doses, with the fourth dose given after the age of four. (If your child is already age four and has had only 2 doses, 3 doses are allowable as long as the last one is given after age four.)
3. MMR (Measles, Mumps, and Rubella) – 2 doses, with the first dose given after the first birthday, and the second dose prior to entry into kindergarten.
4. Hepatitis B Series –3 injections.
5. Lead Screening – a copy of the child’s lead screening record must be presented.
6. Varicella vaccination or proof of natural chicken pox.
In addition, students are required to have a physical exam, and a dental exam should be completed.
