PORTSMOUTH SCHOOL DEPARTMENT

REGISTRATION INFORMATION

 

 

 

The following forms will be required for registration of incoming students:

 

            1.  Registration Form

            2.  Transcript Release Form (if child is transferring from another school)

            3.  Physical Exam Form

            4.  Dental Exam Form

            5.  Home Language Assessment Survey Form

            6.  Childcare Provider Information Form

 

The following documents must be presented at the time of registration:

 

            1.  Birth Verification (one of the following):       

·        Birth Certificate

·        Passport

·        Certificate of Overseas Birth

    The certificates should not be photocopies and must bear an official seal.

    Hospital Certificates are not considered legal documents.

 

2.  An Immunization Record signed by the doctor or his designee.

     Required immunizations include:

            5 DPT

            4 Polio

            2 MMR (after 12 months of age)

            3 Hepatitis B

            1 Chickenpox (or a doctor’s statement verifying that the student

                                     had them naturally.)

    In addition, a lead screening must have been completed.

 

3.  Proofs of residency (two of the following):

·        Driver’s License

·        Rent Receipt with Address

·        Insurance Policy with Address

·        Voter Registration Card

·        Utility Bills

·        Check with Account # Blocked Out

 

4.  Completed Parent Interview Form
